Form questions

Short answers about collection forms, from plan limits, audio and video answers and optional questions to approval, blocked submissions and languages.

Quick answers to common questions about collection forms. Each one links to the article with the full steps.

How many forms can I create?

The number of forms per project depends on your plan: 1 on Free, 3 on Pro, 5 on Premium and unlimited on Business. Paused forms count too.

To create one, see Build a collection form. For more forms, see Plans and pricing.

Can customers record an audio testimonial on my form?

No. Forms don't record audio, so Question Type doesn't offer it. Choose Video Only or Video and Text instead, so customers can record themselves in the browser. See Collect video testimonials.

To add a podcast episode or an audio track that's already online, import it from its link. See Video and audio imports.

How long can a customer's video be?

Up to your plan's limit: 120 seconds on Free, 120 on Pro, 180 on Premium and 300 on Business.

You can set a shorter limit on each question with Max Duration (seconds), or Max Video Duration (seconds) on a Video and Text question. Recording stops on its own at that length. Your plan also limits file size and how many video testimonials you can collect; see Collect video testimonials.

Can I make a question optional?

Yes. Open the Question or Rating page in the form builder and turn off Required, which is on by default. Customers can then move on without answering. The same Required switch on a Consent page decides whether the box must be ticked.

For fields on the About You and About Company pages, and the details of each rule, see Add questions, ratings and required answers.

Can I put my form on my own website?

Yes. Click Share in the form builder's top bar and pick a tab: Inline shows the form inside your page, Modal adds a button that opens it in a window, and Popup adds a round button that floats at the bottom of the page. Copy the Embed Code and paste it into your site's HTML.

The Link tab has the form's address and a QR code, for sharing it anywhere else. See Share or embed your form.

Can I change a form after I've shared it?

Yes. The builder saves your changes as you work, and the live form uses them, so its link, QR code and embed codes keep working. That goes for its look too: see Design your form.

That includes URL slug on the Settings tab. Changing it changes the form's link, and the old address forwards to the new one, so links, QR codes and embed codes you've already shared keep working. The forwarding ends only if another form in the project takes the old slug. See Share or embed your form.

How do I stop a form from taking new testimonials?

Pause it. In the form builder, click the status badge beside the form's name and choose Pause form. On the Forms page, a form's More options menu has Pause form too.

While a form is paused, its link shows a page-not-found error, embedded forms stop opening and the popup button disappears. Choose Activate form to open it again; the link stays the same. See Share or embed your form.

What happens to my testimonials when I delete a form?

They stay in your inbox. Deleting a form permanently removes the form, its pages, its analytics and the other answers customers gave on it, which are the ones under Form answers. Its webhook and delivery history go too. From then on, its link shows a page-not-found error and its embed codes stop working.

The workspace owner and the project's Admins can delete a form: choose Delete in its More options menu on the Forms page, or Delete form in the builder's. If Smart Invites send customers to the form, the confirmation says how many webhooks will stop working, how many routing rules will use their webhook's own form instead, and how many waiting customers won't get their invite email.

Rewards that wait for approval go with the form. A reward that's waiting for you to approve its testimonial can't be sent once the form is deleted. Gift cards that wait are canceled and their amount goes back to the project's reward budget; the confirmation says how many rewards and how much. To send them, approve those testimonials before you delete the form.

A form with no Delete in its menu is the project's default form. It can't be deleted until you choose Set as Default in another form's More options menu.

Deleting can't be undone. To close a form and keep everything, pause it instead.

Do new submissions show on my widgets straight away?

Not until they're approved. New forms have Require Approval switched on in the Settings tab, so each submission waits in Pending for you. To approve well-rated submissions on their own, set Auto-Approve Threshold (optional) there: a submission rated at or above that many stars is approved as it arrives. One without a star rating still waits, and so does one rated at or below the form's Negative Feedback Threshold, whatever the auto-approve setting. For what else a low rating sets off, see Collect negative feedback privately. With Require Approval off, every submission is approved as it arrives.

Once a testimonial is approved, a widget set to Auto-sync picks it up if it passes the widget's rules, and a widget set to Curate shows it only after you add it. See Approve, reject and archive testimonials.

Where do I find the answers customers gave on my form?

In your inbox. The star rating, what the customer wrote or recorded, any images they attached, and their name, job title, company, photo, company logo and website are part of the testimonial. The Phone number, Multiple Choice answers, Private Feedback, Negative Feedback and the Consent tick are kept beside it: click Show more on the testimonial's card and look under Form answers, which only your team sees.

A form's webhook sends those answers too, and the negative feedback alert email includes what the customer wrote. For the full list, see Where each answer ends up.

Why was a customer's submission blocked?

The message the customer saw tells you which check stopped it. A submission your spam check flags isn't blocked: it's saved in Spam (see Keep spam out of your inbox). The one exception is the hidden field meant to catch bots: while Honeypot Field is on in both the form's settings and the project's spam settings, a submission that fills it is discarded and never saved.

  • "Rate limit exceeded. Please try again later." Too many submissions came from the same IP address within the hour, which can happen when people share one office or event network. Raise Max Submissions per Hour, or turn off Rate Limiting, under Spam Protection in the form's Settings tab.
  • "You have already submitted a testimonial with this email." A form accepts one submission per email address. A submission waiting in Spam doesn't count, so a customer it caught can try again. To let people submit again, turn on Allow Multiple Submissions under Behavior in the form's Settings tab.
  • "This form can't accept new testimonials right now. Please let the business know." Your workspace has used all the written, audio and linked media testimonials its plan includes (10 on Free), and you get a notification about it. Upgrade, or archive testimonials you don't show to make room. See Plans and pricing.

If submissions go missing without any message, see Form submissions not arriving.

Can my form be in another language?

Forms have no language setting, but you write most of their words yourself: each page's Title and Button Text, your questions, your answer choices and the consent text. A few built-in words stay in English, such as Back and Submit, the video recorder's buttons and the messages about missing answers, such as "Please share your answer".

See Add questions, ratings and required answers for the page settings.
